Job Description

Position Title: Direct Care Registered Nurse (RN) - School-Based (Special Education / Student Support)

Work Setting: K-12 school environment supporting medically fragile students



Job Summary

Medasource is seeking a Registered Nurse (RN) to provide direct, school-based nursing care for students identified as medically fragile who require an ongoing continuum of nursing support to safely participate in the school setting.

This role involves clinical assessment, care planning, intervention implementation, outcomes evaluation, family and school collaboration, and health teaching in alignment with applicable nursing practice standards and provider orders.


Essential Duties & Responsibilities

1.    Perform ordered nursing procedures in compliance with the applicable state Nurse Practice Act and per physician/healthcare provider orders.

2.    Assess and document the student's physical and psychosocial status, appropriate to the student's age.

3.    Develop and document a plan of care for the continuum of services, collaborating with the Direct Care Nurse Facilitator (or client-designated clinical lead).

4.    Implement and coordinate the plan of care and revise as needs or conditions change.

5.    Evaluate and document the student's response to interventions and revise care approaches to achieve desired outcomes.

6.    Communicate effectively with the student, family, school personnel, and interdisciplinary partners to support care coordination.

7.    Provide and document health teaching to the student and/or family, as appropriate.

8.    Complete role-specific competencies and the student's required skill competency checklist.

9.    Complete orientation requirements and, when assigned, support/mentor newly hired or substitute direct care nurses.

10. Follow client policies and procedures (client name redacted).

11. Perform other duties as assigned.


Required Qualifications

Licensure/Certifications

·      Active, unrestricted Registered Nurse (RN) license in Georgia (or applicable state for the client site).

·      CPR certification (required).

Education

·      Graduate of an accredited school/college of nursing.

Experience

·      3 years of nursing experience providing direct patient care (pediatric experience strongly preferred).

·      Demonstrated experience supporting medically fragile pediatric patients in 1:1 school-based, pediatric home health, outpatient, or inpatient pediatric settings.

·      Experience managing ongoing respiratory support needs, including:

·      Continuous supplemental oxygen management per provider order

·      Pulse oximetry monitoring and documentation

·      Set-up and safe use of portable suction equipment

·      Familiarity with airway clearance techniques, including chest physiotherapy (CPT) or similar ordered therapies

·      Experience managing enteral feeding support, including:

·      G-tube and/or G/J-tube care

·      Continuous feeds via enteral feeding pump

·      Tube site assessment, troubleshooting common alarms/occlusions per protocol, and maintaining ordered venting systems (as applicable)

·      Experience recognizing and responding to seizure activity and related clinical changes, including medication administration as ordered and escalation per emergency plan.

·      Experience providing full-assist care (total care), including toileting support, safe positioning, and transfers consistent with safe patient handling practices.

 

Preferred Experience

·      Prior experience in school nursing (especially PK/elementary settings) and collaboration with multidisciplinary school teams.

·      Comfort responding to cardiopulmonary status changes in pediatric patients and following individualized emergency response plans (per provider orders/school protocol).

 

Skills

·      Strong pediatric assessment skills, including ongoing evaluation of respiratory and cardiac status and timely identification of clinical changes.

·      Competence with common equipment used in continuous-support cases, including:

·      Oxygen delivery systems (portable and/or stationary)

·      Pulse oximeter monitoring and alarm parameter management per order

·      Portable suction set-up, maintenance, and safe use

·      Enteral feeding pump operation and basic troubleshooting

·      Routine care and monitoring of enteral tubes (G-tube/GJ-tube) and site integrity

·      Ability to deliver ordered respiratory therapies (e.g., CPT/airway clearance) and document effectiveness/response.

·      Knowledge of and ability to follow:

·      Provider orders, individualized care plans, and emergency protocols

·      School-based infection prevention practices and safe equipment handling

·      Skilled in medication administration (including via enteral tube, if ordered) and accurate, defensible documentation.

·      Proficiency in providing total care support, including safe positioning and transfers, comfort measures, toileting support, and maintaining dignity in a school environment.

·      Strong communication skills to collaborate with family/guardians, school staff, and clinical leadership, maintaining professionalism and confidentiality.

·      Ability to remain calm and effective in urgent situations, activate emergency response per protocol, and provide clear handoff communication.

 

Physical Requirements

While performing job duties, the RN may be required to stand/walk, negotiate stairs, reach, communicate verbally, manipulate small items, and lift/transport up to 50 pounds over short distances; driving may be required based on assignment.

Vision requirements may include close, distance, and color vision; work conditions may include varying temperature/humidity and confined spaces depending on site needs.
